Instructions on Utilizing Everest University Transcript

Completing the Everest University Transcript form is a straightforward process that ensures your request is handled efficiently. After filling out the form, you will need to submit it via fax, email, or mail, depending on your preference. Ensure all information is accurate to avoid delays in processing your request.

  1. Begin by entering your Student Information. Fill in your last name, first name, and middle initial.
  2. Provide your name as it appeared while attending Everest University.
  3. Include your Student ID Number and the date you are making the request.
  4. Fill in your Social Security Number and the number of copies you need.
  5. List your Telephone Number and an alternate number, if available.
  6. Enter your Date of Birth and provide your signature.
  7. Add your Email Address for any correspondence regarding your request.

Next, you will need to specify where to send the transcript. This can be done by filling out the Mail/Fax To section.

  1. Write the name of the recipient and their street address. If faxing, include the fax number.
  2. Fill in the city, state, and zip code for the first address.
  3. If you need to send a transcript to a second address, repeat the process for the second recipient.

Once the form is complete, send it to the appropriate destination: fax it to 813-902-6782, email it to EUOtranscripts@cci.edu, or mail it to 5701 E. Hillsborough Ave., Suite 2300, Tampa, FL 33610.
